 John Constantine 

John Constantine is a partner in Surgical Management Partners which owns and manages three ambulatory surgery centers and an imaging center in Central Texas. Mr. Constantine also sits on the Board of Advocate MD, a major medical liability insurance company. Mr. Constantine testified before the Texas Legislature on behalf of TASCS during the 79th Legislative Session and has served on the TASCS Board since January, 2005.

 

 William E. Crowder, Jr., M.D., FACOG

William Crowder, Jr., MD helped to start the Conroe Surgery Center in 1983 with 16 other physicians.  After resyndicating and building a new larger facility in 2003, he is currently the Chair of the Board of Managers of that facility, which supports approximately 55 physicians who actively practice there.   He actively practices OB/GYN along with five partners and lives in the Conroe/Woodlands area. Dr. Crowder has been on the HealthSouth Board of Advisors for the past four years.

 

 Linden Dillin, MD

Linden Dillin, MD, is a Board Certified Orthopaedic Surgeon who owns and operates an ambulatory surgery center in Fort Worth.  Dr. Dillin has worked tirelessly to increase revenue for TASCS through increased vendor participation at our meetings and through growing our Facility Membership. Dr. Dillin has served on the TASCS Board since January, 2005.

 

 Michael “Mike” Forman 

Michael “Mike” Forman is a partner with Pain & Anesthesia Associates, a three physician single specialty independent pain group practice in Harlingen, Texas. Formerly from South Florida, Dr. Forman moved to “The Valley” in 1994 to practice both Anesthesia and Pain Management.
Dr. Forman holds a bachelors degree from the University of Florida in Microbiology, and completed his Medical Degree and Anesthesiology/Pain Management training at the University of Miami as Chief Resident. He is formerly Chief of Anesthesia Services at Valley Baptist Medical Center in Harlingen, and currently practices Pain Management full time. He is affiliated with three separate valley ambulatory surgery centers, currently developing a joint venture, multi-specialty, Ambulatory Surgery Center in Brownsville, Texas with Valley Baptist Health System.
Dr. Forman is a member of multiple specialty societies including the American Society of Regional Anesthesia, American Society of Interventional Pain Physicians, Texas Pain Society, American Academy of Pain Medicine, and the Texas Medical Association among others.
He holds certificates from the American Board of Anesthesiology and the American Board of Pain Medicine.

 

 Jack E. Jensen, MD, FACSM

Dr. Jensen is a Board Certified Orthopedic Surgeon distinguished with the title FACSM specializing in the field of Sports Medicine. His orthopedic training was done in the Texas Medical Center and at Eugene, Oregon with prominent leaders in the field. Dr. Jensen has served as the second President of the Texas Ambulatory Surgery Center Society since January, 2005.

 

 Milla Jones 

Milla Perry Jones is the Vice President of Communications and Government Affairs for United Surgical Partners International.  USPI owns and operates 31 ASCs and 7 surgery hospitals in Texas. Ms. Jones has been very active in recruiting Facility Members and is a passionate advocate at both the state and national level for physician ownership of healthcare facilities. Ms. Jones has served on the TASCS Board since January, 2005.

 

 Andrew P, Kant, MD

Dr. Andrew Kant is a board certified orthopedic surgeon who is the founding partner of KSF Orthopaedic Center in Houston. Dr. Kant started KSF Orthopaedic Center in 1976, and is an owner of an ASC and a physician owned hospital. Dr. Kant is a Diplomat of the American Board of Orthopaedic Surgeons. He is also a past president of the Texas Orthopaedic Association, a past president of the Houston Orthopedic Society, the Past President of the Medical staff of Houston Northwest Medical Center where he has also served as chairman of the Department of Surgery. Dr. Kant has served on the TASCS Board since January, 2005.

 

 Bruce Levy

Chief Executive Officer, Austin Endoscopy Center I & II
Dr. Levy received his Medical Degree from Hahnemann Medical College and his Legal Degree from The University of Houston Bates School of Law.
He is a Board Certified Anesthesiologist who previously practiced at The Methodist Hospital in Houston, Texas and served on the Baylor College of Medicine faculty.
He was appointed the Executive Director of the Texas State Board of Medical Examiners in 1993 and served in that capacity until 2000. During that same time, he also served as the Executive Director of the Texas State Board of Physician Assistants, as well as Chairman of the Health Professions Council of Texas.
Dr. Levy has served on multiple, prestigious national medical policy commissions and has been a consultant to a majority of the national medical organizations. He is the past Deputy Executive Vice President of the Federation of State Medical Boards of the United States.
Dr. Levy “unretired” and became the Chief Executive Officer (C.E.O.) of Austin Gastroenterology, P.A. and Austin Endoscopy Centers in 2002.
Dr. Levy has actively testified in front of the Texas Legislature for TMA, TSGE, and on behalf of the ASC industry, as well as in front of the United States Senate Committees on Health Issues.

 

 Dowtin Martin 

Dowtin Martin is a Regional Vice President for Symbion Healthcare which owns and operates a nationwide network of surgery centers providing surgical procedures across many specialties.  Mr. Martin's region includes 6 Symbion facilities in Texas.

 

 Tom A. Michaud 

Tom Michaud is the Board Chairman and Chief Executive Officer of Foundation Surgery Affiliates, which specializes in ambulatory surgery center and surgical hospital development, design, partner recruitment, and operations. Headquartered in Oklahoma City, Foundation Surgery Affiliates is partnered with 22 facilities in Texas, and has another one under development.

 

 Mark Quigley, MBA, CASC

Mark Quigley, MBA, CASC, is the Chief Operating Officer for Atlantic Health Group based in Houston. Atlantic Health Group, which develops, finances and manages ASCs, Hospitals, and Medical Office Buildings.  Mr. Quigley was formerly the Divisional Vice President of Operations for Foundation Surgery Affiliates, former President of the Texas Medical Group Management Association, and has served as TASCS's Vice President since January of 2005.

 

 Vicki  Schultz, RN 

Vicki Schultz, RN is Vice President of Regional Operations for ASCIRA Partners, LLC, where she is in charge of developing and operating surgery centers in Texas and surrounding States. Ms. Schultz has more than 20 years experience as a nurse and health care administrator. Ms. Schultz is a member of ASORN, FASA, and AAASC, and previously served as Regional Manager – Surgery Division for Healthsouth. Ms. Schultz has served on the TASCS Board since January, 2005.

 

 Louis  E. Varela, MD

Louis Varela, MD, is the Co-Founder of the Texas Ambulatory Surgery Center Society and the owner of Mid Town Surgery Center in Houston.  Dr. Varela is also active in charitable service organizations that provide medical assistance to third world countries. Dr. Varela has served on the TASCS Board since December, 2003.

 

 Joe  Zasa, JD 

Joseph Zasa, JD is a partner in Woodrum/ASD and leads Woodrum/ASD's Dallas office where he provides development, operations, management and merger and acquisitions expertise for the firm's ambulatory clients. Mr. Zasa specializes in consummating strategic joint ownership arrangements for the firm's clientele with physicians, hospitals, and strategic partners as well as assisting the firm's clientele in raising capital to finance their ambulatory care requirements.  Mr. Zasa has served on the TASCS Board since January, 2005.
